Required Immunization

All Incoming 8th graders are required to receive a

Tdap and Meningococcal Conjugate vaccine before the 1st day of school.

State requirements for all students has changed for the 2017-2018 school year

If a copy has not been provided prior to the 1st day of school students will not be able to start the first day. The following must be provided and must include the date and exact name of the immunization given. 

  • Tdap
  • Meningococcal  Conjugate

  • Any updated immunization records
  • Updated Action plan (example; students with asthma, food allergies, diabetes, seizures etc.)
  • New for the incoming 2017-2018 school year- Online updated health information with permission for tylenol

Tylenol Policy

*Regular strength Tylenol is available starting in the 6th grade and can be given to a student ONLY with the signed permission waiver from the parent or guardian.  Tylenol will be monitored and parents notified of frequent visits/request for Tylenol.
